Liability Waiver Information:

For the current and for all/any future programs, I, the participant, parent or legal guardian of a participant, agree to protect, defend, reimburse, indemnify and hold Palm Beach County, its agents, designees, employees, elected officials and Golf Professional Services, Inc., its agents, designees, employees, and independent/sub-contractors, Junior Golf Foundation of America, Inc., its agents, designees, employees, and independent/sub-contractors free and harmless at all times from and against all claims, liability, expenses, losses, costs, fines, damages or causes of action of every kind and character, including attorney's fees and costs, whether at trial or appellate levels or otherwise, arising during, as a result of, or in connection with my participation in this program. I hereby assume the risk of participation in this program and agree to hold Palm Beach County, its agents, designees, employees, elected officials, Golf Professional Services, Inc. its agents, designees, employees, and independent/sub-contractors and Junior Golf Foundation of America, Inc., its agents, designees, employees, and independent/sub-contractors free and harmless at all times from and against all claims, liability, expenses, losses, costs, fines, damages or causes of action of every kind and character, including attorney's fees and costs, whether at trial or appellate levels or otherwise, due to their acts, errors or omissions resulting in bodily injury, including death, or damage to my property incident to or in connection with my participation in this program. Photo release: I hereby grant permission to use photographs of me and/or my child(ren) to promote Palm Beach County's park system, Golf Professional services or the JGFA (i.e., promotional printed materials, informational displays, web site, slide presentations, etc.)In connection therewith, I release and hold harmless golf Professional Services, JGFA, Palm Beach County, the Parks & Recreation Department and their agents, servants , sub-contractors or employees from any and all claims and causes of action for circumstances resulting from use of above photographs and/or statements.
